Terms of Service

Effective Date: October 6, 2026

These Terms of Service (“Terms”) govern your access to and use of the website operated by Rev Facial Bar doing business as REV Facial Bar (“REV Facial Bar,” “we,” “us,” or “our”), including online appointment booking, account features, product purchases, memberships, and related digital services (collectively, the “Services”).

REV Facial Bar is located at 15 James P Kelly Way, Middletown, NY 10940.

Please read these Terms carefully. They include a binding arbitration provision and class action waiver for certain disputes. By using the Services, you agree to these Terms and acknowledge our Privacy Policy. If you do not agree to these Terms, do not use the Services.

These Terms govern website use and related transactions. Medical care is also subject to applicable law, professional standards, clinical judgment, informed consent, and separate patient agreements. These Terms do not replace those agreements or limit patient rights that cannot legally be waived.

Eligibility

You must be at least 18 years old and legally capable of entering into a binding agreement to create an account, purchase a membership, or complete a transaction through the Services.

Eligibility for treatment is determined separately by our clinical team. Any permitted treatment of a minor requires the evaluation, authorization, and consent required by applicable law and our clinical policies.

Overview of the Services

REV Facial Bar offers skincare and medical aesthetic services. Through our website and related systems, you may learn about treatments, request or book appointments, purchase products or memberships, and contact our team.

Treatments, providers, products, prices, and appointment availability may change. Booking an appointment does not guarantee that a treatment is medically appropriate for you or that it will be performed.

Changes to These Terms

We may revise these Terms from time to time. The effective date at the top of this page identifies when the current version takes effect.

Changes apply prospectively. Where required by law, we will provide notice or obtain consent before a change takes effect. Revisions will not retroactively alter a pending dispute or an existing purchase or membership agreement without a lawful basis and any required consent.

Your continued use of the Services after revised Terms become effective constitutes acceptance to the extent permitted by law.

Medical Services and Website Information

Medical aesthetic care is provided through appropriately licensed healthcare professionals acting within their authorized scope of practice.

Information on our website, social media, marketing materials, and other public channels is provided for general educational purposes. It does not constitute an individualized diagnosis, prescription, or treatment recommendation.

Browsing our website, submitting a general inquiry, or booking an appointment does not by itself establish a provider-patient relationship. That relationship is established through the appropriate clinical evaluation and care process.

Our providers determine treatment suitability based on your medical history, examination, goals, contraindications, and other relevant factors. They may recommend a different treatment, postpone treatment, or decline to provide a requested service.

You are responsible for providing accurate and complete information about your health, medications, allergies, pregnancy or breastfeeding status, prior treatments, and other information requested by your provider. Follow the preparation and aftercare instructions provided for your treatment.

Treatment benefits, risks, alternatives, and informed consent are addressed separately during the clinical process. Individual results vary, and no specific outcome is guaranteed.

Do not use website forms, email, social media, or text messages for medical emergencies. If you believe you are experiencing a medical emergency, call 911. For nonemergency concerns following treatment, call 845.552.4404 and follow your provider’s aftercare instructions.

Privacy and Health Information

Our Privacy Policy explains how we collect, use, and handle information through our website and business operations.

Where applicable, protected health information and patient privacy rights are addressed in a separate Notice of Privacy Practices provided by the treating medical entity. These Terms do not replace that notice or authorize uses or disclosures of health information beyond those permitted by applicable law.

Use the patient intake process or communication channel designated by our team to provide clinical information. Avoid submitting sensitive health details through public reviews, public comments, social media messages, or other channels not designated for clinical communications.

You must provide accurate, current information. You may submit another person’s information only when you have the necessary authority and permissions.

If you access the Services outside the United States, your information may be processed in the United States as described in our Privacy Policy and subject to applicable law.

Children and International Users

Our website is intended for adults and is not directed to children under 13. We do not knowingly collect personal information from children under 13 through the website. If you believe a child has provided such information, contact hello@revfacialbar.com.

Information needed for any permitted care of a minor must be provided through the authorized patient intake process.

The Services are operated from the United States. If you access them from another country, you are responsible for complying with applicable local law. We do not represent that all offerings are available or appropriate in every jurisdiction. You may not use the Services where prohibited by applicable law or sanctions.

Third-Party Websites and Services

We may link to or use third-party websites and systems, including appointment booking, payment processing, financing, and commerce platforms. These providers may have their own terms and privacy policies, which you should review before using their services.

A link does not imply endorsement. We do not control independent third-party content, availability, or practices.

This section does not exclude responsibilities we have under applicable law, including responsibilities for our own conduct or for vendors handling information on our behalf. It does not classify treating REV providers as unrelated third parties or waive claims arising from clinical care.

User Accounts

If you create an account, provide accurate, complete, and current information. Protect your login credentials, do not share your account, and do not impersonate another person.

Notify us promptly at hello@revfacialbar.com if you suspect unauthorized account access. Use caution on shared devices and sign out when finished.

Digital systems may occasionally be interrupted or unavailable. We may modify, suspend, or discontinue website features, or restrict access for violations of these Terms, security concerns, or other legitimate reasons, subject to applicable law and existing contractual obligations.

Account closure does not eliminate medical record retention obligations, patient rights, or outstanding payment obligations.

Appointments and Treatment Policies

Appointments are subject to availability and confirmation.

Applicable pricing, deposits, cancellation deadlines, late arrival policies, no-show fees, and rescheduling requirements are described in our Appointment and Cancellation Policy and the applicable booking process. Any charges must be disclosed before you agree to them.

Contact our team before booking if you have questions about a policy.

We may reschedule appointments or decline treatment for clinical, safety, staffing, or other legitimate reasons. Refunds and credits will be handled according to the applicable agreement and law.

Memberships and Promotions

Membership benefits, pricing, billing frequency, renewal terms, minimum commitments, cancellation procedures, treatment credits, unused benefits, and refund eligibility are governed by the membership agreement presented when you enroll.

Please review our Membership Terms and your enrollment agreement before purchasing a membership.

Before initiating recurring billing or automatic renewal, we will provide the disclosures and obtain the consent required by applicable law. Cancellation options will be disclosed at enrollment and provided as required by law, including online cancellation for memberships enrolled in online where required.

Promotional offers are subject to the eligibility requirements, expiration dates, and other conditions disclosed with the offer. Promotions do not override clinical eligibility requirements.

Changes to an accepted offer or existing membership will be made only as permitted by the applicable agreement and law.

Payments and Product Purchases

Prices are stated in U.S. dollars unless otherwise indicated. Applicable taxes, shipping charges, deposits, and other fees will be disclosed before payment.

You represent that you are authorized to use the payment method provided and that your billing information is accurate. Payment processors may collect payment information under their own terms and privacy policies.

We may verify transaction information before accepting or fulfilling an order. An automated order acknowledgment does not necessarily constitute acceptance.

If we decline or cancel an unfulfilled purchase after charging you, we will refund the amount paid for the canceled purchase as required by law.

Product availability, delivery estimates, and prices may change for future orders. If an accepted order contains a material error, we will contact you to resolve it, including offering cancellation and a refund where appropriate.

Shipping, returns, exchanges, and refunds are governed by our Shipping and Returns Policy, the terms disclosed at purchase, and applicable law.

Product descriptions do not guarantee an individual treatment or skincare result.

License to Use the Services

We grant you a limited, nonexclusive, nontransferable, revocable license to access and use the Services for lawful personal purposes in accordance with these Terms.

This license does not authorize commercial copying, redistribution, or exploitation of website content. All rights not expressly granted are reserved, subject to rights permitted by law.

Public User Content

If the Services allow reviews or other public submissions, you retain ownership of your content.

By submitting content intended for public display, you grant us a nonexclusive, royalty-free license to host, reproduce, format, and display that content in connection with the relevant website feature.

You represent that you own the content or have the necessary permissions and that it does not violate another person’s rights or applicable law.

This license does not apply to medical records, intake forms, private patient communications, or clinical photographs submitted for care. Marketing use of identifiable patient images, testimonials, or health information requires separate consent or authorization where required by law.

Public submissions may reflect individual opinions and experiences. We do not guarantee their accuracy or endorse every statement. We may remove content that violates these Terms or applicable law.

Nothing in these Terms prohibits lawful, honest reviews or requires you to waive consumer review protections.

Feedback and Suggestions

You may send nonconfidential suggestions about our website or business to hello@revfacialbar.com.

We may use these suggestions to improve the Services without compensation, subject to applicable law. Do not submit confidential material or another person’s intellectual property without permission.

Clinical communications remain subject to applicable privacy protections and are not treated as general business feedback.

User Conduct

You must use the Services lawfully and respectfully. You may not:

  • Impersonate another person or submit fraudulent information.

  • Post unlawful, threatening, harassing, discriminatory, defamatory, or infringing content.

  • Disclose another person’s private information without permission.

  • Send spam or use contact information for unsolicited marketing.

  • Introduce malware, attempt unauthorized access, or bypass security controls.

  • Disrupt, overload, or scrape the website using automated tools without authorization.

  • Copy, sell, or redistribute protected content without authorization, except as permitted by law.

We may investigate suspected violations, remove prohibited content, restrict website access, and respond to lawful requests from authorities, consistent with applicable privacy law.

These actions do not authorize unlawful disclosure of patient information or excuse our own unlawful conduct.

Proprietary Rights

The website and its original text, graphics, photographs, logos, design, and software are owned by REV Facial Bar or their respective rights holders and are protected by intellectual property law.

Website use does not transfer ownership. You may not remove copyright notices or use our branding in a way that implies authorization or endorsement without permission. Rights permitted by law remain unaffected.

Copyright Concerns

If you believe content on our website infringes your copyright, contact:

REV Facial Bar
Attention: Copyright Inquiries
15 James P Kelly Way

Phone: 845.552.4404
Email: hello@revfacialbar.com

Include your name and contact information, identification of the copyrighted work, the location of the allegedly infringing material, and your physical or electronic signature.

Also include a statement of your good-faith belief that the use is unauthorized and a statement that the information is accurate and, under penalty of perjury, that you are authorized to act for the rights holder.

This contact information is for copyright inquiries and does not represent that a statutory copyright agent has been registered.

Electronic Communications

We may provide website and transaction-related notices electronically using the contact information you supply, subject to applicable consent and notice requirements.

Keep your contact information current. Agreement to these Terms alone does not constitute consent to promotional text messages or override your communication preferences or legal rights.

Termination

You may stop using the website at any time. We may suspend or terminate website access for violations of these Terms or other legitimate reasons, subject to applicable law.

Ending website use does not automatically cancel a membership, appointment, or purchase. Use the cancellation process applicable to that transaction.

Termination does not eliminate record retention obligations or legal duties relating to continuity of care. Provisions intended by their nature to survive termination will remain effective, including intellectual property protections and dispute provisions within their stated scope.

Website Warranties

TO THE EXTENT PERMITTED BY LAW, THE WEBSITE AND GENERAL WEBSITE CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE.” WE DO NOT GUARANTEE THAT THE WEBSITE WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE OF HARMFUL COMPONENTS.

TO THE EXTENT PERMITTED BY LAW, WE DISCLAIM IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NONINFRINGEMENT FOR WEBSITE USE.

These disclaimers do not apply to clinical standards of care or exclude warranties, consumer rights, or other obligations that cannot lawfully be excluded. Medical treatments remain subject to applicable professional duties and separate informed consent.

Limitation of Liability

To the extent permitted by law, REV Facial Bar will not be liable for indirect, incidental, special, consequential, or punitive damages arising solely from website access or general website content.

To the extent permitted by law, our aggregate liability for claims arising solely from website access or general website content will not exceed $100.

These limitations do not apply to medical malpractice, bodily injury arising from clinical care, fraud, gross negligence, willful misconduct, unlawful disclosure of protected health information, statutory remedies that cannot be limited, or refunds owed under applicable law or an agreement.

Nothing in these Terms releases healthcare professionals from their clinical duties or limits liability where prohibited by law.

Indemnification

To the extent permitted by law, you agree to indemnify REV Facial Bar against third-party claims and reasonable costs caused by your unlawful misuse of the website, infringement through your public submissions, or material violation of these Terms.

This obligation does not extend to claims caused by our negligence, clinical care, misconduct, or violation of law.

We will provide reasonable notice of a covered claim and will not settle it in a way that imposes an obligation on you without your consent.

Time to Bring Claims

Applicable legal limitation periods govern claims. These Terms do not shorten the time allowed by law to bring a medical malpractice, privacy, consumer protection, or other claim.

Governing Law

New York law and applicable federal law govern these Terms, without regard to conflict-of-laws principles, except where mandatory protections under another applicable law control.

The Federal Arbitration Act governs the arbitration provision to the extent applicable.

Binding Arbitration and Class Action Waiver

If you have a concern about the Services, contact hello@revfacialbar.com or 845.552.4404 so we can try to resolve it.

Contacting us does not suspend a legal filing deadline unless we agree in writing or applicable law provides otherwise.

Except for the exclusions below and where prohibited by law, disputes arising from website use, product purchases, or memberships will be resolved through individual binding arbitration administered by the American Arbitration Association (“AAA”) under its applicable Consumer Arbitration Rules.

Information about the rules, fees, and filing procedures is available at AAA Consumer Arbitration.

FOR DISPUTES COVERED BY THIS PROVISION, YOU AND REV FACIAL BAR WAIVE THE RIGHT TO A COURT TRIAL BEFORE A JUDGE OR JURY AND AGREE TO BRING CLAIMS INDIVIDUALLY, RATHER THAN AS PART OF A CLASS OR REPRESENTATIVE ACTION, TO THE EXTENT PERMITTED BY LAW.

This provision does not apply to medical malpractice, claims of injury arising from clinical care, or disputes that cannot legally be arbitrated.

Either party may bring an eligible individual claim in small claims court. Nothing prevents you from filing a complaint with a regulator or licensing authority or exercising rights that cannot legally be waived.

Fees, arbitrator selection, hearing arrangements, and available remedies will follow the applicable AAA rules and law. We will pay fees allocated to us under those rules.

This provision does not require written-only proceedings or prevent remedies available under applicable law. A court of competent jurisdiction will decide whether an arbitration agreement exists or is enforceable.

If arbitration is unavailable or this provision is unenforceable, a dispute may be brought in a court of competent jurisdiction. To the extent permitted by law, the parties agree to courts serving Orange County, New York, or the applicable federal district.

The class action waiver applies only to the extent enforceable and does not restrict nonwaivable rights.

Mobile Messages

If REV Facial Bar offers a text messaging program and you separately enroll, you may receive appointment reminders, account notices, or promotional messages according to the consent you provide.

Marketing consent is voluntary and is not a condition of purchasing products, memberships, or treatments. Automated technology may be used where disclosed during enrollment and permitted by law.

Message frequency varies. Message and data rates may apply, and your carrier’s charges are your responsibility.

Message delivery is not guaranteed. Do not rely on texts for emergencies or time-sensitive medical concerns.

To opt out, reply STOP to the number that sent the message or use an unsubscribe method supplied with the message. For assistance, reply HELP where supported or email hello@revfacialbar.com.

You may also contact us to request that messages stop. We will honor revocation requests as required by law, and you may receive a confirmation of your opt-out.

Provide a number you are authorized to use and notify us if it changes. Wireless carriers are not responsible for delayed or undelivered messages.

Our Privacy Policy describes applicable data practices. Agreement to these Terms alone does not enroll you in a messaging program.

General Provisions

These Terms, together with the policies and agreements applicable to a particular transaction, govern website use and related transactions.

If a separate signed patient, membership, or purchase agreement addresses a specific subject, that agreement governs that subject to the extent permitted by law. Mandatory legal protections always control.

If a provision is unenforceable, the remaining provisions remain effective unless applicable law requires otherwise. Failure to enforce a provision is not a waiver.

You may not transfer your contractual rights without our written consent, except where permitted by law. We may assign these Terms in connection with a lawful business transfer, subject to existing obligations and applicable law.

Website use does not create a partnership, employment, or agency relationship. This does not negate a provider-patient relationship established through clinical care.
